Day Services - DH

Responsibilities

Provides training and supervision in various activities of daily living including: self-help, communications, interpersonal relations, leisure activities, etc. helping each individual to live more independently. Provides the necessary supports and services to achieve Individual Day Services Plan goals and outcomes. Observes all policies and procedures including, but not limited to, the handling of incidents, emergencies, operation of agency vehicles, handling of individual's funds and petty cash. Provides written and/or oral feedback to appropriate supervisory staff regarding individual or facility-related concerns. Fulfills all record keeping and documentation requirements including daily/weekly progress notes, and assists with the monitoring of the Day Services for each individual assigned. Will assist in developing and maintaining community resources, volunteer options and ensure integrated opportunities for the individual as outlined in the ISP / Day Service Plan. Serves as positive role model for individuals. Interacts with others in a way which will assist individuals in developing skills necessary for successful community living. Maintains neat appearance, hygiene and grooming. Promotes positive behaviors and interactions with all individuals.

Qualifications

High school diploma or equivalent with up to 2 years of experience, or an Associate degree in human services filed. Must meet Lifetime Assistance Incorporated’s Vehicle Operator Requirements.  The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

Must successfully complete all required training.  Must be able to perform the following responsibilities including but not limited to: First Aid/CPR, SCIP (Strategies for Crisis Intervention and Prevention) and Medication Administration. Must be able to lift, move and transfer up to 50 pounds. Subject to background checks including but not limited to being fingerprinted/criminal history record check, NYS Child Abuse Registry, OPWDD Staff Exclusion List and Mental Health Law 16.34.

Typical schedule for the position is 8a-4:30p with some flexibility
