How do I apply for services?

Who is eligible for services?
Anyone diagnosed with a developmental disability prior to the age of 21.

What is a developmental disability?
Developmental disabilities are a variety of conditions that become apparent during childhood and cause mental or physical limitation. These conditions include autism, cerebral palsy, epilepsy, mental retardation and other neurological impairments.

Is Lifetime Assistance Certified by the State?
Yes. All of our sites are reviewed and certified by NYS Office of Mental Retardation and Developmental Disabilities.


Where are you located?
Lifetime Assistance, Inc. is headquartered in Chili, New York in southwest Monroe County. To visit Lifetime Assistance, see the following instructions or call us at (585) 426-4120, ext. 3100 and we will gladly provide directions.

Are employees trained?
Yes. All of our employees go through an extensive training program initially upon hire and on a regular basis thereafter.

How are services paid for?
Most of our service recipients receive funding through Medicaid and/or Supplemental Security Income (SSI). If your family member meets the eligibility requirements, we can assist you in this area.

Is there a waiting list? 
Yes, however each persons specific needs and requests are evaluated for the best possible options.  Our Intake Committee will discuss various options that may be applicable to your specific situation.
